An investigation has found a man was most likely hit and killed by a train because he was visually impaired.
The incident occurred in July 2009 at the Newmarket West station in Auckland.
The Transport Accident Investigation Commission says the 69-year-old was walking along the platform when he veered into the side of a moving train.
It says he fell through the gap between the train and the platform edge onto the track below and was fatally injured.
The investigation said the cause of the accident was likely to be related to the man's impaired vision, distraction or a combination of both.
It said the design of the platform met the New Zealand and international guidelines for assisting vision-impaired and blind people.
