An investigation is under way into how the capital's mental health service handled the case of a woman who stabbed five people after she was discharged from Wellington Hospital.
The woman was yesterday found not guilty by reason of insanity of five charges, including murder, assault with a weapon and wounding with intent to cause grievous bodily harm.
A 67-year-old man was killed and four others injured in the knife attacks in suburban Johnsonville last July.
The woman was assessed by members of the crisis team at the hospital's emergency department earlier the same day.
Thinking they had persuaded the woman to take her medication, she was discharged.
Capital and Coast District Health Board mental health service head Nigel Fairley said a panel of three independent and internal experts had reviewed the case.
They were currently preparing their recommendations, he said.
