Report into man's death outlines failures

A report by the Health and Disability Commissioner has pointed to systematic failures in dealing with a 69-year old man who died at Whangarei Hospital.

The report by commissioner Anthony Hill says there were delays in treating the man, who was admitted to Whangarei Hospital in 2009 with suspected coronary problems.

The man was admitted in the early hours of a Saturday. The next week he began to deteriorate, collapsed and died.

The Health and Disability Commissioner says the consultant who examined the man failed to consider alternative diagnoses and breached the Code of Health and Disability Services Consumers' Rights.
