Man given 10 times prescribed dose

A coroner is recommending that a Christchurch hospice review its medication education for staff, after the death of a man who was given 10 times the medication prescribed.

Geoffrey Alfred Haase, of Christchurch, died on 7 November last year while at the Nurse Maude Hospice, four-and-a-half hours after being over-dosed.

Coroner David Crerar said the cause of death of the 73-year-old was prostate cancer, but a 25 milligram dosage of oxycodene, when only 2.5mg was prescribed, would have brought forward Mr Haase's inevitable death.

Mr Crerar recommends that Nurse Maude reviews its medication education, and for all nurses to review their practice during busy shifts.
