Manjit Singh, 47, is facing charges over an attack on a 50-year-old woman with a ceremonial sword last month.

The ministry said Mr Singh was undergoing community-based treatment supervised by the Auckland-based Mason Clinic, a forensic mental health service, at the time.

Ministry of Health director of mental health John Crawshaw said he was under a compulsory care order and had breached the conditions of his leave.

Dr Crawshaw said the inquiry would make recommendations, and was expected to be completed by mid-2016.

Mr Singh is now in the Waikato District Health Board's Henry Rongomau Bennett Centre, awaiting trial.
