Former Atletico captain Fernando Torres put Chelsea ahead in the 36th minute in the semi-final second leg at Stamford Bridge.
But an equaliser from Adrian just before half-time was followed by a Diego Costa penalty on the hour mark, for Samuel Et'to's rash challenge in the box, before Arda Turan put the tie beyond reach in the 72nd minute.
The 3-1 aggregate scoreline follows the dour scoreless draw last week in Spain and sets up the all-Madrid final in Lisbon later this month, after Real astoundingly thrashed Champions League holders Bayern Munich 4-0 on Wednesday in Germany, for a 5-0 aggregate win.
Chelsea's 41-year-old Australian goalkeeper Mark Schwarzer became the oldest player ever to start a Champions League match.
