Federer comfortably beat Italy's Fabio Fognini in straight sets to give the Swiss an unassailable 3-1 lead.
Switzerland, who lost to the United States in their only other Davis Cup final appearance, will play France in the November final.
Italian Andreas Seppi edged Michael Lammer in the remaining dead rubber to make the final score 3-2.
