A new report identifies a number of safety problems that led to a small plane with 13 people on board failing to take off and ending up with its nose in a nearby garden.
The incident, in which no one was injured, involved a Great Barrier Airlines plane at Pauanui in October 2011.
The Transport Accident Investigation Commission's report says the main reason for the aircraft failing to get airborne was that its centre of gravity was significantly outside the permissible limits.
It says the pilot's technique was an issue, and the plane was also too heavy for the available runway length.
The commission says the standard of pilot training and supervision at Great Barrier Airlines was below that required, and it has asked the Civil Aviation Authority to address the safety issues.
