State Highway 3 near Otorohanga in King Country has reopened following a fatal crash at 1.45pm on Monday.
One woman died on impact and the driver of another vehicle received moderate to serious injuries.
Police say it appears a southbound car crossed the centreline and collided with a northbound utility.
A detour was put in place as police and emergency services attended the crash scene.
